Indications

According to official monographs:

• All forms of parkinsonism.

• Drug-induced extrapyramidal symptoms (e.g., from neuroleptics) including acute dystonia, akathisia, rigidity. 

• In some cases, poisoning scenarios: nicotine poisoning and organophosphate (OP) compound poisoning. 

Posology and Administration

• Adults: For extrapyramidal symptoms: 2.5 to 5 mg (i.e., 0.5-1 mL) IM or slowly IV. If necessary, same dose may be repeated after 30 minutes. 

• For severe parkinsonism: initial 10-20 mg (2-4 mL) divided over the day. 

• Children/adolescents: Doses require adjustment e.g., up to ~1 mg (~0.2 mL) for very young children (up to 1 year) in extrapyramidal reactions. 

• Older patients / impaired hepatic/renal: Starting with lowest possible dose; there are limited data in severe impairment—use caution. 

• Administration notes: Use IM or slowly IV. After opening an ampoule, any unused portion must be discarded.
